Please note: If you have already filled out the Northwest Health Unit's vaccine registration form, please do not wait for a code to book. The system has changed and when you are eligible, book online at www.ontario.ca/bookvaccine

If you cannot book an appointment through the provincial system:

No Ontario Health Card: An Ontario health card is required to book using the online system. If you do not have one, you can still get the vaccine but will need to phone 1-866-468-2240.

Unable to leave home: If you are unable to leave your home due to illness or impairment and want to get the vaccine, phone 1-866-468-2240.

Aged 16 or 17: Those aged 16 or 17 who want to receive the Pfizer vaccine can call 1-866-468-2240. We will take your name and add you to our list for future clinics.

Non-residents of Ontario:

Dose 1: COVID-19 vaccinations are offered to all individuals who live, work, go to school or plan an extended visit beyond 16 weeks (to ensure they can get their second appointment) in the Province of Ontario. The vaccine will be administered to these individuals in accordance with the phased vaccination rollout.

Dose 2: If you have already had your first dose of the vaccine before entering our region and are now due for your second dose, you will soon be able to book a 2nd dose appointment through the Provincial Vaccine Booking System. More information will be coming soon. Most people are due for their second dose 16 weeks after their first. We will require proof of your first dose to give you a second dose.
